Key Responsibilities:
• To provide skilled nursing care to clients within their homes in accordance with physician’s directives and care plans.
• Monitor patient health, administer medications, and coordinate treatments.
• Educate clients and families on disease processes, progress, medications, and care techniques.
• Maintain accurate clinical records and collaborate with the wider health care team.
• Maintain confidential medical and staff records and implement policies and procedures.
• Ensure compliance with current healthcare regulations, medical laws, and local business laws.
• Collecting information from potential clients, or relatives for booking
• Clinical assessment for each new client and review as necessary
• Making individualized care plans.
• Prepare monthly medication administration records for each client and adjust as necessary
• Providing Client and Family support
• Visiting clients biweekly
• Offering advice and emotional support to patients and their families
• Monitoring clients in home records and maintaining those in office
• Generating daily reports on client’s status and general matters
• Conduct Health Screenings and Health Education sessions
• Ensure clients insurance policies cover requisite care and liaise with finance department
• On call duties for weekly intervals
• Ensure compliance of policies and procedures for the company
• To manage complaints (clients/caregivers) and ensure report to supervisor
• Hiring of caregivers jointly with the clinical team and providing guidance as necessary
• Client Development
• Ensure New and Renewal Insurance Applications are prepared and submitted on time
• Schedule maintenance
• Supervise Caregivers
• Staff Training/ Inservice Education
• Provide a weekly report to management
• Liaise with other members of the health team re the clients
• And any other related duties
